Community Health Worker

Job Description

Community Health Workers (CHW) are frontline public health workers who have a close understanding of the community they serve. CHW assist individuals and families to adopt healthy behaviors utilizing an evidenced based educational program. This trusting relationship enables the CHW to serve as a liaison/link between health/social services and the community. The CHW provides information with available resources, provides social support and informal counseling, advocates for individuals and community health needs. The CHW will collect data to help identify community health needs. He/She will conduct outreach for the Healthy Start Initiative, through the Indiana Rural Health Association, based out of Daviess Community Hospital, to implement programs in the community that promote, maintain, and improve individual and community health. This positions reports directly to our Perinatal Navigator.

Qualifications

- Minimum of a high school diploma or its equivalent with an associate degree preferred

- Experience in a health field such as but not limited to working with children in day care, medical office or school that provides a medical or community support service

- Must be 18 years of age

- Knowledge of some medical terminology preferred

- Must have reliable transportation and a valid driver's license

- Ability to maintain CPR certification and Universal Precaution training on annual basis

- Experience working in a multi-cultural setting

- Ability to initiate and maintain positive working relationships with DCH and IRHA staff and other organizations

- Ability and willingness to provide emotional support, encouragement and motivation to patients

- Ability to follow all county personnel policies and rules of the organization
