With FollowMyHealth® you can manage your health information and communicate with providers in a secure, online environment – 24 hours a day / 7 days a week. Once you create your account, you will be prompted to search for and connect with available providers in your area.

The FollowMyHealth® patient portal is an online tool that gives you anywhere, anytime access to your personal health records. This allows you to take a proactive role in managing your care.

Daviess Community Hospital now has a new texting (mobile experience) for our patients. Access to your health information through the patient portal will not change.

For all clinic patients, you’ll now receive texts for:
  • Appointment reminders
  • Post Care summary (After checkout, you’ll receive a text message from us. Simply click the link and enter your last name and date of birth to access your visit summary. You’ll have 96 hours to view. After that, your information will be available through your FollowMyHealth account.)
  • On Demand Messages (when providers have important notifications/updates to share)
  • Visitor survey
Patients discharged from a hospital stay or visit in the ER will receive texts for:
  • Welcome message
  • 24 hr follow up
  • Post Care summary
  • Visitor survey
