Welcome to Diabetes & Nutrition Education at DCH, where our mission
is to help you embrace a healthier lifestyle—one meal, one choice
at a time. Our experienced Registered Dietitian Nutritionists (RDNs) offer
personalized, one-on-one counseling for diabetes management, heart health,
weight loss, and more. We understand that adopting new habits can feel
overwhelming, which is why we tailor our guidance to your individual preferences,
goals, and medical needs.
The Expertise of Our Registered Dietitian Nutritionist (RDN)
A Registered Dietitian Nutritionist has completed rigorous education and
training accredited by the Accreditation Council for Education in Nutrition
and Dietetics (ACEND). This background ensures our RDN can deliver evidence-based
Medical Nutrition Therapy (MNT) to support your journey toward better health.

loss, food allergies, and intolerances.
Insurance Coverage for Medical Nutrition Therapy (MNT)
Many insurance plans, including Medicare Part B, cover MNT for diabetes
and kidney disease. Under Medicare Part B, you may be eligible for up
to three hours of MNT during your first year, and two hours each subsequent
year. If you have private insurance, contact your provider to confirm
your coverage for conditions such as heart disease, diabetes, and obesity.
Getting Started: Referrals & Appointments
A referral from a physician or nurse practitioner is required to schedule
an appointment for Diabetes & Nutrition Services. Your provider’s
office can refer you by completing our
Diabetes and Nutrition Education Referral Form.
