When a condition affects the brain, spinal cord, or nerves, everyday tasks can feel harder. At Daviess Community Hospital’s CORE Center, our neurological rehabilitation team helps people rebuild mobility, balance, speech, swallowing, and independence—one meaningful step at a time. We deliver coordinated physical therapy (PT), occupational therapy (OT), speech-language pathology (SLP), and vestibular rehabilitation in our outpatient clinics in Washington and Odon, Indiana, welcoming patients from Daviess County and across Southern Indiana.
Our goal is simple: create a plan that fits your life—then adapt it as you improve.
We personalize care for a wide range of neurological diagnoses and symptoms, including (but not limited to):
Not sure if we treat your condition? Call (812) 254-8889—we’ll help you determine next steps and whether a referral is needed for your insurance.
We review your history, medications and timing, current symptoms, and goals. Your therapist screens strength, balance, movement quality, arm/hand use, dizziness/vestibular function, speech, cognition, and swallow.
Together, we set clear goals (e.g., safer walking, fewer falls, stronger voice, better memory strategies) and choose the right services. Your plan includes in-clinic therapy and a doable home routine scaled to “good days” and “low-energy days.”
We track outcomes (walking speed/distance, balance scores, hand function, vocal loudness, attention/memory measures) and share updates with your medical team. Your plan evolves as you do.
We’re continually expanding our neurological rehabilitation services. Here are some of our current services:
Don’t see your condition? Ask us. Our team treats a broad range of neurologic needs and will guide you to the right provider or service.
Experiencing new neurologic symptoms? Call 911 or your physician immediately. Once medically cleared, therapy can help you recover safely.

Do I need a referral?
Many insurers require a physician referral. Call us and we’ll help
you determine next steps.
How often will I come to therapy?
Frequency varies by diagnosis and goals. Some patients benefit from a focused
episode of care; others prefer periodic “tune-ups.”
Can you coordinate with my neurologist?
Yes. With your permission, we share progress and recommendations so your
medical and therapy plans work together.
Is aquatic therapy an option?
Often, yes—when medically appropriate. Our therapy pool includes
a hydraulic lift chair for safe entry/exit.
