What it honors: Extraordinary compassion and clinical excellence by nurses. The DAISY
Award is an international program that recognizes nurses who go above
and beyond.
When presented: Quarterly (April, July, October, January).
Who can nominate: Patients, families, community members, and DCH staff.

What it honors: Teammates (all staff except licensed nurses) who uphold and strengthen
our work and our workforce—supporting excellent care, access, education,
and community health in tangible ways.
When presented: Quarterly (April, July, October, January).
Who can nominate: Patients, families, community members, and DCH staff.

Yes. Please submit a separate nomination for each individual.
Can I nominate a team?
Our awards recognize individuals. If a group was involved, describe the
nominee’s unique contribution.
What if I don’t know the full name of the person I want to nominate?
Share as much detail as possible—department, location, shift/timeframe,
and what occurred—and we’ll work to identify the teammate.
