Every three years, Daviess Community Hospital conducts a Community Health Needs Assessment (CHNA). It’s a vital process that allows us to hear directly from the people we serve — and this year, your voice matters more than ever.
By taking our 2025 CHNA survey, you help us:
Identify and prioritize the most urgent health concerns in Daviess County and surrounding areas.
Discover barriers to care and gaps in local health services.
Strengthen programs for vulnerable and under-represented populations.
Build solutions that truly reflect your needs and values.
