Kentucky Derby Fundraiser
For 2018, the DCH Foundation is looking to raise funds to help support
a new space for our growing Oncology Services! The first opportunity to
show your support will be by contributing to and attending our 4th Annual
Kentucky Derby Fundraiser. This event will provide you the opportunity
to enjoy a great meal, live viewing of the Kentucky Derby, as well as
music and entertainment. All of the funds raised by this event will be
directed toward the building of a new space to help better serve our community.

Whether you are a patient or a family member of a patient who wants to

Include us in your Estate or Financial Planning arrangements – A planned gift is any gift, given for any amount and for any purpose,
which is made through estate or financial planning. Making appropriate,
significant, charitable gifts generally requires the assistance of a professional
legal or financial planner. If you have already included Daviess Community
Hospital Foundation or Daviess Community Hospital in your estate planning,
this form and mail it to the Foundation Office. It is important to us that we meet
our donors' expectations and give them the recognition they deserve
