A discharge planning nurse is available to help you and your family plan for the care that might be needed after leaving DCH. The discharge planning nurse can refer you to other agencies for help in your own home and can assist in making arrangements for transfer to other living arrangements, i.e. nursing homes and adult foster care homes. You can ask for this help directly or have your doctor or family make a request for you.
