Home Health Services are available to provide continuity of care from hospital stay to home recuperation. Home Health Services are arranged by the discharge planner upon physician referral. When referrals are received prior to the patient's dismissal, a Home Health staff nurse will contact you with an explanation of services. For more information on types of services, contact the Home Health Department at 254-8950.
