| | The DCH Diabetes Management Center, located at 300 N.E. 14th Street in Washington, provides specialized education, support, and care for those with diabetes.
Our mission is to inform, empower, and support all individuals with diabetes and their family members and caregivers. And, through our community outreach services, we reach out to find those people who are at high risk for diabetes and its complications.
The Center is recognized by the American Diabetes Association for quality self-managment education and provides expert resources for diabetes management, from dietary counseling to insulin pump therapy. The Center provides services to all ages and works closely with area physicians and nurse practitioners to meet patient needs.
Services:
- Comprehensive Diabetes Education Classes three 2-hour sessions that teach patients how to self manage their disease.
- Individual Sessions for people who have already had classes or need more one-on-one instruction.
- Meter and Insulin Instruction for people who are new to monitoring their blood sugar or insulin.
- Pump Starts for people who are interested in going on an insulin pump and intensively managing their diabetes.
- Nutrition Consults for anyone who needs to be on a special diet or meal plan (like weight loss, low protein, low fat, low cholesterol, or any restricted diet).
Physician Referral: A referral by a physician or nurse practitioner is required for participation in all programs.
Payment: Medicare pays 80% for classes. Medicaid and private insurance companies vary by type. Please call (812)254-2250 for more information and/or to schedule an appointment.
