Here is a listing of benefits available to Daviess Community Hospital staff members.
   






Paid Time Off (PTO)








.

Eligible employees are those working a minimum of 40 hours per 2-week pay period. If full-time (40 hrs/wk), one would earn approximately 23 days per year for the first 4 years. Beginning the fifth year, one would begin accruing at a higher rate, which would earn them approximately 28 days per year. If one is working only 20 hours per week, the accrued hours of PTO will reflect this.







Long-Term Disability








.

Eligible employees are those working a minimum of 40 hours per 2-week pay period. The monthly rate is based on the annual income of the employee. The payments of 66.6% of the employee’s regular pay will begin shortly after 90 days of time off due to the employee's disability. Premium withheld last pay of the month only.




Short-Term Disability








.

Eligible employees are those working a minimum of 32 hours per week. The monthly rate is based on the plan you select. Coverage begins the first day of an accident and on the eighth day of an illness.







Bereavement Leave








.

Bereavement Leave is granted to full and part time employees. Full time employees are those working between 64 and 80 hours per pay period. Part time employees are those working between 40 and 63 hours per pay period. Full time employees will receive 3 paid days off for the death of an immediate family member. Immediate family members are defines as mother, father, step parent, guardian (proof of guardianship must be presented), spouse, children, step children, sister, brother, step sister, step brother, grandchild, and parent-in-law. One (21) day will be granted for the death of a grandparent, sister-in-law, brother-in-law. Part time employees will be given (1) day off, with pay, in the case of death of an immediate family member.







Health Insurance

Eligible employees are those working a minimum of 30 hours per week. Current monthly rates are as follows:




  • Employee only...............$143.44 ($500 deductible) $97.10 ($1000 deductible)


  • Employee/Child(ren)....$290.18 ($500 deductible) $194.18 ($1000 deductible)


  • Employee/Spouse........$347.52 ($500 deductible) $242.74 ($1000 deductible)


  • Employee/Family..........$387.26 ($500 deductible) $254.86 ($1000 deductible)



    Premiums include a $100 deductible Pharmacy card







    Dental/Vision Insurance








    .

    Eligible employees are those working a minimum of 32 hours per week. Current monthly rates are as follows:




  • Employee only.................$41.15


  • Employee/Family.............$101.07





    Group Critical Illness





    .

    Eligible employees are those working a minimum of 32 hours per week. This insurance will pay you directly on the onset of a Covered Critical Illness. Premiums are based on the plan you select. You may purchase critical illness coverage in amounts from $5,000 to $50,000. The lump- sum benefit is paid directly to you upon the initial diagnosis of a covered critical illness and will be paid to you regardless of any other insurance you may have.

    Cancer Coverage








    ..

    Eligible employees are those working a minimum of 32 hours per week. Cancer Select is a cancer only policy that provides benefits for the direct medical and indirect non-medical costs of cancer treatment.








    Retirement (401a)








    .

    Eligible employees are those working at Daviess Community Hospital for 1 year and a minimum of 1000 hours during that year. The hospital contributes 1.5% of yearly gross wages to the plan.







    Pension/Employer Match (403b)








    .

    Eligible employees are those working a minimum of 1000 hours per year. Employee is eligible to contribute beginning date of hire. After one year of service DCH will match a portion of the employee’s contribution up to 4% contribution.







    Life Insurance








    .

    Eligible employees are those working a minimum of 32 hours per week. The life insurance, provided to you at no cost, is equal to one's annual salary up to $35.000. Additional term life can be purchased by the employee on themselves, spouse, and/or children.







    Whole Life Insurance








    .

    After 6 months of employment, all employees are eligible for voluntary contribution.







    Weekend, Shift, and Holiday Differential








    .

    All employees are eligible. The amount paid is 10% for each differential.







    Spending Accounts









    Healthcare and dependent care spending accounts are available through the Section 125 Cafeteria Plan. This money is set aside pre-tax.







    Dietary Discounts








    .

    All employees are eligible.







    Tuition Reimbursement








    .

    All employees are eligible after 6 months of employment. Employees are reimbursed 80% for books and tuition.







    Win-Win Program








    .

    All employees are eligible.







    Employee Recognition and Incentive Programs








    .

    All employees are eligible for recognition based on years of service and this recognition is done annual . The Employee Incentive Program is a program that rewards full and part time employees for various employee participation examples and achievements. In return, employees earn points that may be redeemed for gifts. See the handbook in Human Resources for additional information.








    Employee Assistance Program








    ..

    All employees and their immediately family are eligible upon hire. Eight sessions are made available per person in the family per situation. All employees are eligible.









    Transplant Coverage








    ..

    Employees who carry the hospital's insurance are covered by organ transplant insurance for a $1,000,000 benefit. The coverage includetransplantnt related expenses incurred 10 days before and 365 days after the date of the transplant.
